General Information

Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff. If you wish to obtain a copy of your personal health record, or part thereof, please ask the receptionist or doctor. You will then be asked to fill in a form regarding this.

We welcome your feedback to help us improve our service. We have a Suggestion Box available. If you have not had your complaint dealt with to your satisfaction, you have the right to call the Health Care Complaints Commission on 9219 7444.

Patients should consult with the doctor prior to prescriptions or referrals being issued. We will not, for medico-legal reasons, backdate referrals, so please check that you have a valid referral with your specialists before you see them. Medical certificates will only be issued with consultations.

In most cases, results will not be given over the phone, except in the event of a TeleConsult. When an investigation is performed, e.g. x-ray or blood tests, we advise patients to make an appointment to discuss results. If normal, Pap smear results will be mailed to your preferred address.
